Patient level programmes
There are a variety of reasons for PCTs to develop patient level management programmes, for example:
- to streamline the full patient journey to achieve waiting times targets
- to respond to local health factors such as a significantly higher than average prevalence of a specific disease
- to collect better data to inform the commissioning cycle (for example relative referral rates and referral patterns)
- to provide improved patient care for specific diseases through better integration and coordination of all the various providers (including social services)
- to release resources for other uses through the proactive management of high risk/high cost patients (for example using the PARR model)
